Beyond the Headwinds: The Medicare Advantage Market Outlook for AEP 2026

Key takeaways

  1. More than half of eligible beneficiaries already choose Medicare Advantage, and that share keeps rising. These headwinds and more are shaping the market outlook for this enrollment season.
  2. Compliance, retention, and post-enrollment engagement are headwinds that matter more than ever in the Medicare Advantage market outlook this AEP.
  3. Appropriate and ethical cross-selling of life insurance and annuities can deepen client relationships and expand your practice.

Here’s a number worth pinning above your desk this AEP: 93 million Americans are projected to rely on Medicare by 2060. It’s true the industry has real challenges ahead. Carriers are tightening margins, and quality is under the microscope. But if all you hear is the word “headwinds,” you’re missing the bigger story. Progress is supposed to be difficult. It’s never impossible.

“We’re in the people business first and foremost,” says Scotty Elliott, Chief Distribution Officer, Health at AmeriLife. It’s about cultivating meaningful relationships every day, and that’s the currency of our industry and of our business.”

What does the Medicare Advantage market outlook look like through 2034?

The short answer is growth. Medicare covered 69.3 million people in 2025, according to an analysis of the 2026 Medicare Trustees Report. Medicare Advantage now enrolls 35.2 million people, or 55% of eligible beneficiaries. The Congressional Budget Office projects that share will climb to 63% by 2034.

Longer lives and an aging population continue to fuel demand. The Medicare population is projected to grow from 68 million in 2024 to 93 million by 2060, giving anyone serving this market decades of runway.

Medicare carriers rely on independent licensed insurance agents

Independent distribution plays a growing role in how people choose coverage. Brokers helped 44% of first-time Medicare Advantage enrollees in 2022, up from 36% in 2014. Carriers understand they need strong distribution partners to succeed.

A growing market, a fundamental need, and a proven distribution model add up to a strong foundation for any business plan.

From growth to quality: AEP 2026 Medicare Advantage is shifting

Medicare Advantage saw unprecedented growth from 2020 to 2024. Now carriers are focused on managing margin, and “grow at all costs” has given way to profitability. Quality is the theme at nearly every AEP rollout this year, so your game plan should prioritize:

  • Compliant marketing and customer engagement
  • Post-enrollment customer journeys, not just the sale
  • Strong placement, persistency, and retention
  • Low complaints to Medicare (CTMs)

What is AmeriLife’s Your Quality Partner initiative?

AmeriLife is preparing to announce Your Quality Partner, an enterprise-wide Medicare Advantage initiative built around four key business drivers: marketing, training and education, member engagement, and compliance. Each driver will carry measurable goals, so AmeriLife can hold itself accountable to being known as a quality partner across the industry.

“We want to make sure we underscore each key business driver with some goals and objectives that are measurable,” says Scotty. “So, we can hold ourselves accountable to being known as a quality partner throughout the industry.”

A holistic approach can grow your Medicare practice

AmeriLife’s long-standing vision is to help licensed insurance agents serve as trusted sources of information and support for beneficiaries, not just Medicare plan specialists. Client-focused practices are already proving the model with recent wins across the company’s nationwide distribution network from producers that have leveraged their strong relationships to

Your Medicare clients already trust you. Ask what their other financial needs are and bring in a financial professional or wealth partner when the situation calls for it.

The leadership mindset that helps you win AEP 2026

Start with a simple formula: E + R = O, or Event plus Response equals Outcome. You can’t control every event, but you can always manage your response. Leaders who live “above the line” choose ownership, accountability, and intentionality. Those below it blame, complain, and defend.

Three daily resets can help you stay above the line:

  1. Rise and shine, not rise and whine
  2. Get to, not have to
  3. Get better, not bitter

Remember that inaction breeds doubt and fear, while action breeds courage and confidence. Want more insight into the Medicare Advantage market outlook, AEP road show highlights, and leadership strategies for the season ahead?  Watch Scotty’s episode of the Marketing Masterminds podcast.
