AmeriLife and its affiliates have an interest in ensuring that its business partners comply with applicable rules and regulations, including the Federal Telephone Consumer Protection Act (“TCPA”) and related rules and regulations, Do-Not-Call (“DNC”) requirements, and Centers for Medicare and Medicaid Services (“CMS”) requirements, when applicable.

This questionnaire is designed to help us understand the level of your company’s control environment related to these important regulatory considerations. To be able to partner with AmeriLife, you must complete this questionnaire, and have it certified by an individual(s) at your company who is knowledgeable about the subject matter below.
