Last month, AmeriLife announced that associates would be required to complete a mandatory COVID Questionnaire the first day they return to the office and on the 1st and 15th of each month going forward.

We have streamlined the procedure. Associates will now only need to complete the COVID Questionnaire one time – on their first day back in the office.

Associates will be asked a modified version of the questionnaire as they enter the building each day for work.

You will find the COVID Questionnaire in Workday as a notification. (The small bell icon in the upper right of the Workday homepage.)

Again, this should be completed on your first day back in the office and not in advance.  

PLEASE NOTE: the COVID Questionnaire is a mandatory form, and failure to complete will result in an automatic unpaid leave until the form is completed.
